July 1 - Replacement of Lost or Stolen Disabled Parking Permits Will Require Certification by Medical Provider

Beginning July 1, a change in Florida law requires that a medical provider complete a certificate of disability on a Parking Permit Application (form HSMV 83039) for anyone needing to replace a lost or stolen disabled parking permit. The certificate of disability cannot be more than 12 months old and will affect both replacement permanent (blue) and temporary (red) disabled parking permits.

Tax Collector Ken Burton Jr. recommends that applicants mail their Parking Permit Application (form HSMV 83039) to Manatee County Tax Collector, P.O. Box 25300, Bradenton, FL 34206-5300. There is no fee for the replacement disabled parking permit.

Individuals who do not need to replace their disabled parking permit are not required to submit anything at this time. However, beginning October 1, 2012, this new requirement will apply to anyone renewing a disabled parking permit.
