In accordance with Section 322.245(5), Florida Statutes, courts may notify the Florida Department of Highway Safety and Motor Vehicles (FLHSMV) to suspend the driver record of anyone failing to pay financial obligations related to any criminal offense (non-traffic) other than those specified in Section 322.245(1), Florida Statutes. This is not a D6 suspension. Suspensions/sanctions for failure to pay court financial obligations are indefinite until the court requirements are met – hardship licenses cannot be issued for a Fail to Pay Court Financial Obligation Suspension.

Follow the steps below to have your Florida driver license/privilege reinstated.

1. Obtain Information about Court Requirements

Contact the Clerk of Court in the county to which you owe financial obligations to obtain information about satisfying the court requirements. Some courts may allow payments online or by phone. If you don’t know which county you owe fees to, visit mydmvportal.flhsmv.gov/home/en/publicweb/dlcheck to locate this information.  You may also contact the Florida Department of Highway Safety and Motor Vehicles (FLHSMV) customer service center by phone at 850.617.2000.

2. Satisfy the Court Requirements

To satisfy the court requirements you must either: Once one of these court requirements are met, you will be provided with an Affidavit to Reinstate Driver License/Privilege (form HSMV 74010) by the Clerk of Court.

Note: The Clerk of Court may electronically mark your suspension(s) as Requirements Met. If so, you may be eligible to reinstate your record online. (See Step 3 below.) Verify that your suspension(s) are marked as "Req. Met" on mydmvportal.flhsmv.gov/home/en/publicweb/dlcheck.

The affidavit(s) must list the case number and be dated within the last 30 days. If more than 30 days has passed, a new affidavit must be obtained. Multiple case numbers from the same court can be listed on one affidavit; however, if the cases are from different courts, an affidavit is required from each court with the appropriate case number(s). Multiple case numbers could be an indication that more than one sanction has been placed on your record.

3. Reinstate Your Driver Record

Once the steps above have been completed, you have two options to reinstate your driver record:

Option 1: Save time and money! Many suspensions can be cleared online - or scan the QR code to access the site. (In order to clear online, the Clerk of Court must have marked your suspension(s) as Requirements Met as detailed in Step 2.) Remitting payment and reinstating online will save you a trip to our office.

Option 2: Visit the Driver License Office at 904 301 Blvd W, Bradenton. (Note: An appointment is required for all transactions.) You must present the Affidavit(s) to Reinstate Driver License/Privilege (form HSMV 74010).

An Online Application is REQUIRED for all transactions! To complete your driver license or ID card transaction at one of our offices, you must complete the Online Driver License & ID Card Application (https://taxcollector.jotform.com/210103795736152) – or scan the QR code to access the application – prior to your appointment. Do not print the application; an associate will access your electronic submission during your appointment. Applications are valid for 90 days.

We will accept cash, check, debit cards, and credit cards. A $2.50 processing fee for debit cards and a 2.5% processing fee (minimum of $2.50) for credit cards will apply. Our office does not retain any portion of this fee.
