Replacement

If your license has been lost, stolen or destroyed, you can obtain a replacement license at our office.

• You must provide your concealed weapons license number. If you do not know your license number, you can obtain it by calling the Department of Agriculture at 850-245-5691.
• A notarized statement explaining that your license has been lost, stolen or destroyed is required. Include your full name, date of birth, address, and telephone number.
• For changes to your name, please provide a copy of your marriage certificate, divorce decree or court documentation.

IMPORTANT: A replacement license with a name change requires review by FDACS and cannot be printed from our office.
